Care Co-ordinator

£18,720 - £22,000 pro rata, per annum (plus enhancements) depending on experience.40 hours a week, (part time considered - pro rata).

The Care Coordinator will assist with the development of Support At Home plus, our CQC registered domiciliary care service currently operating in Dartford, Gravesham and Swanley.

The Care Coordinator will be professional, dynamic and proactive in ensuring the services provided by Alzheimer’s & Dementia Support Services meet CQC expectations and the highest possible standards, whilst maintaining client confidentiality and dignity at all times.

The successful applicant will play an integral part of the team and will work within a busy and vibrant office. The ability to work within a fast paced, dynamic and good humoured environment is a must.You will have:

  • Experience in the domiciliary care industry.

  • Familiarity with the Dartford/Gravesham/Swanley areas

  • NVQ level 3 preferable and/or work towards

  • Good working knowledge and experience of Microsoft Word and Excel.

  • Experience with using Rostering Software.

  • Experience with using "Staff Plan" Rostering software, or the willingness to learn how to use it.

  • Organised and good customer service skills.

  • Willingness to undertake care calls in the community.

  • Ability to undertake On Call duties during weekdays and weekends on a roster basis.

  • Flexibility to work late or start early depending on requirements of the business.

  • A full UK driving license, and access to a vehichle

  • Residence within reasonable commutable distance to Dartford/Gravesham/Swanley area.

Main duties will include:

  • Accepting, allocating and processing referrals for new work efficiently.

  • Roster geographically to maximise utilisation of resources.

  • Accepting and processing changes and amendments to existing care packages.

  • Assigning care workers to Service Users, whilst working within the standards and requirements of the company.

  • Ensuring adequate cover is available for sickness, absenteeism or holidays.

  • Work with the Registered Manager to pre-empt recruitment needs

  • Working with other members of the care team to ensure high quality service provision.

  • Liaising with Service Users and workers to ensure an efficient service provision.

  • Keeping accurate records on the system and working with the care team to ensure all data is up to date and relevant.

  • Preparation of reports as required by the Social Services and CQC.

  • Ensuring care worker rosters are completed on a timely basis for the week.

  • You will be paid for on call duties and business mileage in addition to your basic salary.

Confirmation of all appointments is subject to proof of Eligibility to Work in the UK, satisfactory Enhanced DBS clearance, satisfactory references and medical clearance.
